Job Description

Provides quality, efficient and effective patient care in accordance with standards/guidelines, applicable statutory requirements and accepted clinical practice for the nurse practitioner/physician assistant profession. Is engaged in consulting and advising treatment plans for patients and following established protocols. Work includes providing direct patient care by examining, diagnosing and analyzing the patient's current health condition relative to cardiac care. Responsible for developing short- and long-term goals and treatment plans, in collaboration with a physician, and/or established protocols/standards. Maintains familiarity with the patient throughout the inpatient/outpatient treatment plan and tailors care and education to meet the needs of each patient based on clinical guidelines. Is financially responsible for all aspects relating to transcription/coding.

Nurse practitioner will provide autonomous care to patients with Heart Failure, Coronary Disease, Valvular Dysfunction and those requiring device therapy. The NP will play an essential role in expanding the BDHS Heart Failure program and promoting a multidisciplinary program.

The role of the Cardiology NP includes (but is not limited to):
Assessment of health status through obtaining a relevant health history and performing a comprehensive physical exam
Identify health needs and referring to appropriate health care providers where required
Recognize early clinical features of worsening HF and implement appropriate interventions to prevent hospital admissions
Prevention and/or management of any complication - Initiate appropriate diagnostic tests
Optimize medical therapy using American College of Cardiology(ACC), American Heart Association(AHA)/ Heart Failure Society of America(HFSA) clinical guidelines for HF as the standard of care
Promote self-management strategies through individualized education of patients (families, caregivers and health care personnel)
Follow patients recently hospitalized with HF( New York Heart Class III-IV), frequent re-hospitalizations and those with multiple, active co-morbidities. Track readmissions.
Discuss end of life care/management in patients . Assist in promoting Pallative Care management.
Coordinate care in collaboration with cardiologist those patients with chronic poor quality of life; need for intermittent or continuous IV support; and those considered for assist devices as destination therapy.
Act as a resource for and work in collaboration with Cardiopulmonary Rehab. Utilizes the program in order to enhance coordination of patient care.
Provide ongoing continuing education to medical and nursing staff.
Lead a developed multidisciplinary Heart Failure Team during monthly team conference -
Develop strategies to improve patient education and adherence to medical regimens -
Continue home and nursing home visits if required

REQUIREMENTS

EDUCATION

Required:

Masters of Nursing or Doctorate of Nursing Practice, with board certification as Family Nurse Practitioner

Preferred:

N/A

LICENSURE and CERTIFICATION

Required:

Active Montana state RN and APRN licenses
Montana APRN prescriptive authority
DEA license
Current board certification from ANCC or AANP as FNP
BLS through AHA as healthcare provider
ACLS

Preferred:

N/A

EXPERIENCE

Required:

1-2 Year of demonstrated competency in the skills and knowledge pertinent to the practice of a Family Nurse Practitioner for all patient populations in primary and/or urgent care

KNOWLEDGE OF:

Personal computers, harware and basic software programs including email, word processing and spreadsheets
Electronic Medical Record documentation

SKILLS IN:

Communicates effectively verablly and in writing
Demonstrates competency in the organization and presentation of instructional and educational material
Excellent time management skills

ABILITY TO:

Ability to form effective working relationships with other employees, management, physicians and patients
Refer to policy manual for specific instructions/guidelines.
Follows established policies and procedures.

Follows patient throughout the inpatient/outpatient course.
Consults with multi-disciplinary team as appropriate.

25% Time Spent -
Coordinates patient /family education and self care: tailors education to meet the needs of each individual patient

5% Time Spent -
Is recognized as an expert nurse and clinical leader within the specialty of cardiology and acts as a resource for medical and mursing staff.
Provides continuing education as needed.
